Youth Transition Program
This program offers support to teens and young adults who are transitioning from Department of Children and Families (DCF) oversight to independent adulthood. Services include:
- Personal Futures Planning - Informal goal setting and person-centered planning tools help young people envision their futures and identify the steps for turning dreams into realities.
- Case Management - Guidance through the process of applying for and accessing services from local and state agencies including programs offered through the Dept. of Developmental Services, Dept. of Mental Health, Massachusetts Rehabilitation Commission, Dept. of Transitional Assistance, Social Security, local and regional housing authorities, financial assistance programs, and so forth.
- Assistance with Essential Life Skills - To help each individual find or maintain an appropriate living situation, and manage household tasks, personal finances, hygiene and appearance, achieve positive habits for maintaining good health through appropriate exercise and nutrition, social skills development, community connections, decision-making and pursuit of educational goals.
- Job Placement, Training and Support - Assistance with skills assessment and career interest inventory, resume preparation, job search, assistance in completing applications forms and preparing for interviews, liaison with employers and/or assistance with application to other job assistance programs
