Individual Home Supports




Individual Home Support Services focus on assisting individuals to participate in their community while leading lives with choice and opportunity. Individuals reside in their own independent settings.

Person centered goals and priorities are the cornerstone of the program. Developing relationships, increased control over life choices, and development of skills needed to achieve goals are explored and focused on daily.

While determining individual’s needs, a variety of skills assessments are conducted. Services are then developed through a person-centered planning process, the Individual Support Plan (ISP) in partnership with the Department of Developmental Services.

Goals are based on what each individual wants to accomplish as well as needs identified in the skills assessment.

The team supports adults develop the skills, confidence, and social network necessary to live on their own with 5 to 25 hours of weekly staff support.

  • Support accessing medical, dental, and health needs with community-based providers.
  • Assistance with entitlements such as Social Security, food stamps, MassHealth, Medicaid, fuel assistance, and rent subsidies.
  • Money management assistance to develop the skills to manage funds whenever possible. The United Arc can also serve as the representative payee.
  • Recreational activities to bring opportunities to develop social skills, confidence, and a close social network.
  • Meal planning and Nutrition
  • Emergency prevention

The monthly program fee covers support services only, including all services available.  All other living costs, including rent and food, are paid separately.
